YOUR FIRST VISIT
Once you set up your appointment, here are some tips to remember:
- Come 10-15 minutes early so that you will get back for your appointment with the doctor on time.
- The entire first visit (typically a 30 minute appointment slot) will be dedicated to a detailed evaluation of you or your child.
- Please bring any important documents that you have with you (list of medications, x-ray/MRI reports, police/accident reports, etc.)
- For minors or infants please know their SS# and mothers labor and delivery signification history if possible.
- You may fill our the main intake form here to make things faster. Please print and fill this form out completely.
- If you recently had an injury from a motor vehicle accident, please also fill out both sides of the "Personal Injury Questionnaire" (and be as detailed as possible). It is very important for proper documentation and the multiple parties that may be involved. Form Side #1 / Form Side #2
